Brain aneurysm clipping
ব্রেইন অ্যানিউরিজমAn aneurysm is a weak, ballooned spot on a brain artery. If it bursts it bleeds around the brain (subarachnoid hemorrhage). A ruptured aneurysm must be secured quickly because a second bleed is often fatal.
Common symptoms
- Sudden, severe headache (হঠাৎ তীব্র মাথাব্যথা)
- Vomiting, neck stiffness, fainting
- Drooping eyelid or double vision
- Seizure or loss of consciousness
When surgery is considered
- A ruptured aneurysm
- Unruptured aneurysms with high risk features (size, shape, location)
How Dr. Zico treats it
In aneurysm clipping, a tiny titanium clip is placed across the neck of the aneurysm under the microscope, closing it off while keeping the artery open. Some aneurysms are treated from inside the vessel (coiling); the choice depends on the aneurysm's shape and location.

AVM (arteriovenous malformation)
AVMAn AVM is a tangle of abnormal vessels where arteries connect directly to veins. It can bleed or cause seizures, often in young people.
Common symptoms
- Bleeding in the brain
- Seizures
- Headache
When surgery is considered
- AVMs that have bled and can be removed safely
How Dr. Zico treats it
AVM treatment is chosen from surgery, endovascular treatment and radiosurgery, alone or combined, depending on size and location.

Brain bypass (EC-IC bypass)
ব্রেইনের বাইপাস অপারেশনHeart bypass is well known; the brain can have a bypass too. In EC-IC bypass an artery from the scalp is joined to a brain artery to bring in extra blood when a main artery is blocked or must be closed to treat a complex aneurysm.
Common symptoms
- Repeated mini-strokes from a blocked artery with poor blood flow
- Moyamoya-type narrowing
- Complex aneurysms needing the parent artery closed
When surgery is considered
- Selected patients after detailed blood-flow studies
How Dr. Zico treats it
Dr. Zico has performed 16 EC-IC bypass operations. Bypass needs fine microsurgical stitching of vessels about 1 to 2 mm wide.
EC-IC bypass surgery, often called brain bypass surgery, is done for a small group of carefully selected patients.

Common symptoms
- A mini-stroke (TIA): brief weakness, numbness or loss of speech
- Brief loss of vision in one eye

- Significant carotid narrowing with symptoms

The artery is opened, the plaque removed and the artery closed, under careful blood-flow monitoring.
Carotid endarterectomy lowers the risk of a future stroke when the artery is badly narrowed.

Most strokes are treated with medicines. Surgery is needed for bleeding from an aneurysm or AVM, some large bleeds or swelling that threaten life, and for carotid narrowing or poor blood flow in selected patients.
Clipping or coiling for an aneurysm?
Both are effective. The choice depends on the aneurysm's shape, size, location and the patient's condition. Some aneurysms are better suited to clipping, others to coiling.
