Hydrocephalus: VP shunt, ETV and NPH
হাইড্রোসেফালাস: ভিপি শান্ট ও ETVFluid builds up when its flow is blocked (for example by a tumor or a narrow channel) or when it is not absorbed. In babies the head grows too fast. In adults pressure rises and causes headache and drowsiness. Normal pressure hydrocephalus (NPH) in older adults develops slowly.
Common symptoms
- A baby's head growing fast, eyes looking down (শিশুর মাথা বড় হওয়া)
- Headache and vomiting, worse in the morning
- Drowsiness or blurred vision
- In older adults: unsteady walking, memory problems and loss of bladder control (NPH)
When surgery is considered
- Hydrocephalus that is increasing on scans or causing symptoms
- Blocked flow suitable for ETV
- NPH that improves after a test tap of fluid
How Dr. Zico treats it
ETV (endoscopic third ventriculostomy) uses a thin camera to make a new opening for the fluid, so no tube is left in the body. When ETV is not suitable, a VP shunt operation places a thin tube with a valve that drains fluid from the brain to the abdomen. Arachnoid cysts are opened endoscopically or with microsurgery.

Chiari malformation
কিয়ারি ম্যালফরমেশনIn Chiari malformation the lowest part of the brain (the tonsils of the cerebellum) sits too low, crowding the opening at the base of the skull. It can block fluid flow and cause a fluid cavity in the spinal cord (syrinx).
Common symptoms
- Headache at the back of the head, worse on coughing or straining
- Neck pain, dizziness or unsteadiness
- Numbness or weakness in the hands
- Swallowing difficulty in some patients
When surgery is considered
- Symptoms clearly linked to the Chiari
- A syrinx in the spinal cord
How Dr. Zico treats it
Chiari malformation surgery (posterior fossa decompression) makes more room at the base of the skull so fluid can flow normally. Dr. Zico has performed 26 of these operations.
Chiari malformation treatment starts with watching mild cases; Chiari malformation surgery is for symptoms or a syrinx.

সাধারণ প্রশ্ন ও উত্তরETV or VP shunt: which is better?
ETV avoids a permanent tube and its long-term problems, but it works only when the fluid is blocked in a suitable place and is less reliable in young babies. A VP shunt works in almost every type of hydrocephalus but may need revision later. Dr. Zico recommends one after reading the MRI.
