Neck disc, cervical spondylosis and myelopathy (ACDF)
ঘাড় ব্যথা, ঘাড়ের ডিস্ক ও রগে চাপWear and tear in the neck (cervical spondylosis) or a cervical disc prolapse can press on a nerve going to the arm, or on the spinal cord itself. Pressure on the cord is called cervical myelopathy. It tends to worsen slowly and is the main reason neck surgery is advised.
Common symptoms
- Neck pain spreading to the shoulder and arm
- Tingling or numbness in the hand (ঘাড় ব্যথা ও হাত ঝিনঝিন)
- Clumsy hands, dropping things, trouble with buttons
- Unsteady walking or weakness in the legs
When surgery is considered
- Signs of cervical myelopathy on examination and MRI
- Arm pain that does not settle with treatment
- Increasing weakness in the arm or hand
- OPLL (ossified ligament) narrowing the canal
How Dr. Zico treats it
Most neck disc problems are treated from the front with ACDF (anterior cervical discectomy and fusion). Dr. Zico performs ACDF with an operating microscope and has done more than 350. When there is myelopathy he fixes the level with a plate and screws to lower the chance of the problem returning. Some cases are better treated from the back, with microforaminotomy or lateral mass screws; OPLL and Hirayama disease need individual planning.
ACDF surgery in Bangladesh is now a routine, well-proven operation. Cervical disc prolapse surgery and cervical myelopathy surgery are planned only after cervical spondylosis treatment with medicines and exercise has been considered. Posterior cervical foraminotomy suits some patients with arm pain from one nerve. OPLL surgery and Hirayama disease treatment are planned individually.

Spondylolisthesis: open and minimally invasive (MISS) surgery
কোমরের হাড় সরে যাওয়াIn spondylolisthesis one vertebra slips forward on the one below. The slip narrows the space for the nerves and makes the spine unstable, which causes back pain and leg pain on walking.
Common symptoms
- Back pain that is worse when standing or walking
- Leg pain, heaviness or numbness after walking a short distance
- Tight hamstrings or a change in posture
- Pain relieved by sitting or bending forward
When surgery is considered
- Leg symptoms that limit walking despite treatment
- A slip that is increasing on X-ray
- Nerve weakness
- Instability seen on flexion and extension X-rays
How Dr. Zico treats it
Surgery frees the trapped nerves and fixes the slipped level with screws and a spacer (fusion). Dr. Zico has done 74 of these as open operations and 33 as minimally invasive spine surgery (MISS), including MISS TLIF, where the screws are placed through small cuts in the skin. MISS usually means less muscle damage and a quicker return to walking, but not every patient is suited to it.
Spondylolisthesis treatment starts without surgery. When an operation is needed, lumbar fusion surgery can be done open or as minimally invasive spine surgery; Dhaka patients often ask for MISS spine surgery, and Dr. Zico explains whether it suits them.

Spine fracture, spinal cord injury and the craniovertebral junction
মেরুদণ্ডের হাড় ভাঙা ও আঘাতFalls from roofs and trees and road accidents are common causes of spine fractures in Bangladesh. A broken or unstable spine can injure the spinal cord. The craniovertebral junction (CVJ), where the skull meets the top of the neck, needs special care because the cord there controls breathing and all four limbs.
Common symptoms
- Neck or back pain after a fall or accident
- Numbness or weakness in the arms or legs
- Loss of bladder or bowel control
- Neck pain with a tilted or fixed head (possible atlanto-axial dislocation)
When surgery is considered
- An unstable fracture on CT or MRI
- Pressure on the spinal cord or nerves
- Atlanto-axial dislocation or odontoid fracture that will not heal on its own
- Increasing deformity
How Dr. Zico treats it
The aim is to take pressure off the cord and hold the spine steady with screws and rods so the patient can sit up and start rehabilitation early. Dr. Zico's work here includes 39 atlanto-axial dislocation operations, odontoid and hangman fracture fixation, 360 degree neck stabilisation and 34 dorso-lumbar fixations.
Spine fracture surgery and spinal cord injury surgery are emergency work. Atlantoaxial dislocation surgery, odontoid fracture treatment and other craniovertebral junction surgery need special skill because of the cord at the top of the neck.

Children's spine: meningomyelocele and tethered cord
শিশুদের মেরুদণ্ডের জন্মগত সমস্যাMeningomyelocele is a form of spina bifida: a baby is born with a sac on the back that contains part of the spinal cord. Tethered cord means the lower end of the cord is stuck and stretches as the child grows.
Common symptoms
- A swelling or sac on a newborn's back (নবজাতকের পিঠে ফোলা)
- Weak legs or a foot deformity
- A tuft of hair, dimple or mark on the lower back
- Bladder problems or back pain in an older child
When surgery is considered
- An open meningomyelocele (repair is done within the first days of life)
- Tethered cord with worsening legs, bladder or pain
- Hydrocephalus that often comes with meningomyelocele
How Dr. Zico treats it
Meningomyelocele surgery closes the sac and protects the cord from infection. Tethered cord surgery releases the cord. Many of these children also need treatment for hydrocephalus, which Dr. Zico manages as well.
Meningomyelocele surgery and spina bifida treatment are done soon after birth; tethered cord syndrome surgery is usually done later in childhood.

Thoracic disc and other spine conditions
পিঠের মাঝখানের মেরুদণ্ডের সমস্যাDisc prolapse and calcified ligaments (ossified ligamentum flavum) are less common in the thoracic spine, but when they press on the cord they cause stiff, weak legs. Spinal arachnoid cysts are fluid pockets that can do the same.

- Pain in the middle of the back or around the chest
- Stiff or weak legs, numbness below the chest
- Unsteady walking

- Spinal cord compression on MRI with symptoms

No. Most people improve with medicines, posture correction and physiotherapy. Surgery is advised when the spinal cord is compressed (myelopathy), or arm pain and weakness do not settle.
Should I see a spine specialist or a neurosurgeon in Dhaka?
